On-site Fee Schedule:

New patients $250.00 for the first half hour and $75.00 for each 15 minute increment after that.
Established patients, $200.00 for the first half hour and $75.00 for each 15 minutes increment after that.

We now offer an exclusive Pre-payment Plan:

If you select this plan you will be able to pay for services in advance and receive 10% more services for no additional charge. These fees will apply for one year for any individual, family or corporation. Any increment remaining will roll over to the next year with continued enrollment in this plan. The plan is available in $250.00 increments starting at $500.00.

·        $500.00 guarantees $550.00 in services

·        $1000.00 guarantees $1100.00 in services and a MI 5 card with your records already saved on it (established patients only)

·        $2000.00 guarantees $2200.00 in services and two MI 5 cards (records loaded for established patients)
